Not a bug, no scroll made from spell traces exist for badge which is why it take you to the equipment trace window (KMS doesn't have a badge with potential).
Use a chaos scroll, chaos … (more)scroll of goodness or incredible chaos scroll of goodness to scroll your Ghostship badge then you can star force it. (less)

It is no longer possible to take 1 damage from normal monster anymore by having excess amount of defense alone.

hammering out the rest of the slots. Now it says "NUMBER OF HAMMER APPLIED: 2(MAX)". Wtf does that mean if I pure clean slate it with spell trace, I'll lose the hammer slots...

Probably just the default text when an equipment has no more hammer available.
Since it is the only equipment I heard of in GMS that can be hammered more than twice, the text was probably hard coded to say 2 no matter how many times you can hammer it since the developer (KMS) assumed there will never be a equipment with more than 2 hammer slots. You should be able to clean slate the slots back no problem.

Brief bug summary:
… (more)Monster attacks that deal multiple lines of damage fails to trigger invincibility if the top line out of all the damage line is a miss/guard. This leaves character to be vulnerable to get hit by more attacks than intended leading to death if the player is unlucky enough to have all of the top line of damage to be guard/miss in a short time span. For example if 6 monster attack at once with their multiple damage line attack, and the top line of each attack is a miss/guard, then you will take damage from each and single one of those attacks at once.


Steps to reproduce:
Get hit by a monster that deal multiple lines of damage in one attack (example of monster shown in video are Advanced Knight A, Official Knight D, Mutant Ribbon Pig, Reindeer, Ghostwood Stumpy, ...)
"+% second invincibility when attacked" potential is optional and not needed to see the bug on a brand new character.
